This form needs to be completed every time you board a pet with us.
Please fill out this form if you are admitting your pet
into our hospital for lab testing (urine or bloodwork), bath, x-rays, or other scheduled appointment..
Please fill out this form if you are a new client at EBAH or if your address
and information has changed and we need to update your record.
Please fill out this form if your pet is being admitted into our
hospital for a "problem" exam not scheduled at a particular time of the day with a doctor.
This may include shaking of head, limping, skin conditions, vomiting, diarrhea, etc. or
other scheduled appointment.
Please fill out this form if your pet is staying the day with us. No other exams are needed,
no bath is needed, etc.
Please fill our this form if your pet is scheduled for a dental cleaning.
Please fill out this form if your pet is scheduled for an annual physical exam with a doctor.
Please fill out this form if your pet is scheduled for surgery.
